As a Home Health Aide, you’ll be seen for the value you bring to our care team. This is a multifaceted role where you can use your clinical expertise, critical thinking skills and consultative care abilities to make a tremendous difference for our patients.

RESPONSIBILITIES:

• Delivering personalized care to patients in their own homes.
• Administering care plans.
• Evaluating the patient’s progress.
• Providing consultation to patients and their families.
• Providing detailed reports to the rest of the care team.

EXPERIENCE AND TRAINING REQUIREMENTS:

• High school diploma.
• CNA certification.
• Experience working in a long-term, rehabilitation or home care setting.
• Valid driver’s license and current automobile insurance.
